If we decide to work together, you’ll complete an extensive medical symptom questionnaire and share previous lab results. I’ll gather this information from you 3-days before your intake session ensuring I can review it behind the scenes making the most of our 1-on-1 time together.
We’ll review your assessment and discuss my recommendations for lifestyle changes and potential additional testing.
We’ll meet every other week for 30 minutes over 3 months to track progress and adjust your plan as needed.
At the end of 3 months, choose to continue with optional check-ins every 1-2 months or proceed on your own.
I ask that you reschedule your sessions at least 3 days in advance. I extensively research your unique circumstances based on your intake forms and information from our previous sessions. Sometimes this can take up to several hours. To provide you with the most benefit, I should do this no more than 3 days before your intake session.
I will not be able to maximize the benefit of your intake session if I am unable to prepare properly. If you do not submit your information 3 days before your session, your session will be rescheduled.
You are welcome to take notes during our sessions together, though all information provided will be summarized and sent to you via email within 24 hours after completion of the session.
Accepting cash only, allows me to keep the cost of my services low by requiring less time or additional staff for insurance billing.
Each insurance company is different, just as each body is different. Please contact your insurance company to find out how to submit your receipts and after-visit summaries for reimbursement. ICD billing codes will be included in your after-visit summary and receipt.
The easiest way to contact me is through your patient portal in Practice Better. There you will be able to ask questions or submit comments. You can expect a response within 48 hours. You can also leave a message at 715-204-9318 and expect a return email, also within 48 hours. If you are experiencing urgent symptoms, please present to your local urgent care or emergency department.
